Modification of identification data

Modification of identification data – shall be performed if the personal data, data from act of identity or contact data are modified. In case when the name/surname of the owner is modified, the procedure of Reissue of card from the reason of Owner name/surname modification shall be performed. The card Owner shall present the act of identity, the document which certifies the name/surname modification, where are indicated the previous name and surname (according to the current data, registered by the Bank and on the card), as well as the new name and surname, which correspond with the data from the act of identity of the Owner. In dependence of the name/surname modification reason, such documents may be:

  • Certificate of marriage;
  • Certificate of divorce;
  • Decision of the Office of Civil Status regarding the name/surname modification.
